Tissue Receiving & Accessioning SOP
Purpose
To ensure every specimen is received, identified and registered accurately with full traceability.
Scope
All histopathology specimens received by the laboratory.
Responsibilities
- Reception technician: verify, log and accession
- Lab manager: monitor rejections and turnaround
Materials & equipment
- Requisition form
- LIS / accession register
- Container labels
- Rejection log
Procedure
- Check specimen container and requisition are labelled with at least two identifiers (name + ID).
- Confirm the specimen matches the request; check fixative is present and adequate.
- Assign a unique accession number and label the container.
- Record date/time of receipt and fixation status in the LIS/register.
- Apply sample-rejection criteria for unlabelled, leaking, or inadequately fixed specimens; document and notify the sender.
- Route accepted specimens for grossing.

Safety
- Wear gloves and lab coat; handle formalin in a ventilated area.
