Fixation SOP
Purpose
To standardise fixation so morphology and antigenicity are preserved.
Scope
All specimens requiring formalin fixation.
Responsibilities
- Grossing technician / pathologist: ensure adequate fixation before processing
Materials & equipment
- 10% neutral buffered formalin
- Appropriately sized containers
Procedure
- Place tissue in 10% NBF at a fixative-to-tissue ratio of about 10:1.
- Slice large/thick specimens (bread-loaf) to allow penetration.
- Record time in and out of fixative.
- Ensure adequate fixation time appropriate to specimen size before processing.
- Minimise cold-ischaemia time; fix promptly after removal.

Safety
- Handle formalin in a fume hood with PPE; follow the formalin handling SOP.
