Wrong reagent concentration prepared
Also known as: incorrect reagent strength, reagent dilution error
Slides may show unexpected staining intensity or lack of staining altogether. Tissue sections may appear overly pale or excessively dark.
Where it usually appears: Typically appears during routine staining procedures or when preparing solutions for immunohistochemistry.
- Incorrect measurement of reagent
- Improper dilution technique
- Use of expired reagents
- Misinterpretation of concentration guidelines
- Verify the concentration of reagents used
- Check the dilution calculations
- Inspect the reagent expiration dates
- Prepare a new reagent solution at the correct concentration
- Re-run the staining procedure with the corrected reagent
- Document the error and results for quality control
- Implement a double-check system for reagent preparation
- Regularly train staff on proper dilution techniques
- Maintain a log of reagent preparations and concentrations
